Membership Fees
Ages 0 - 17 $29/month with one parent membership $49/month without one parent membership
Ages 18 - 39 $69/month
Ages 40 - 64 $79/month
We will be accepting membership of Medicare-age patients in the near future.
Haven Direct Primary Care does not accept any insurance.
Family discount | $20 per additional child with one parent and one child full membership
One-time office visit for non-members | $180 (There may be an additional fee for procedures)
Sports Physicals | $29 (free for members)
DOT physicals | $120 (free for members)
Medical Marijuana Card Evaluations | $129 (free for members)
Medical ear piercing | $49 ($29 for members)
Re-enrollment Fee:
There is no enrollment fee. However, if a member should desire to rejoin the practice after terminating membership, there shall be a re-enrollment fee of $149.00.
Free home visits within a 5 mile radius of the practice. Additional charge for a home visit outside of this radius.
